Civil Registry
Birth, Marriage, Death Certificates
- Go to the Local Civil Registry Office (LCRO) in we municipality.
- Fill out the request/application form for the specific certificate.
- Present valid identification and supporting documents (e.g., hospital record for birth, marriage contract, death certificate signed by physician).
- Pay the required processing fee at the cashier.
- Submit the receipt and form to the LCRO staff.
- Wait for the processing period (usually 3–5 working days).
- Claim the certificate at the releasing section.
Business Permits
New, Renewal, Closure
- Visit the Municipal Business Permit and Licensing Office (BPLO).
- Secure and fill out the business permit application form.
- Submit requirements (DTI/SEC registration, barangay clearance, lease contract or land title, etc.).
- Undergo assessment and inspection (fire, sanitation, zoning).
- Pay the assessed fees and taxes at the Treasurer’s Office.
- Present official receipt to BPLO.
- Receive the business permit (usually within 1–3 working days).
Building Permits
Construction, Occupancy, Inspection
- Go to the Municipal Engineering Office.
- Fill out the building permit application form.
- Submit required documents (lot title, tax declaration, barangay clearance, architectural/structural plans signed by licensed engineer/architect).
- Pay the corresponding fees at the Treasurer’s Office.
- Wait for evaluation and inspection by the Engineering Office.
- Receive the approved building permit.
- For occupancy permits, request inspection after construction completion, then secure clearance before use.
Barangay Clearance
For Employment, Business, Community Transactions
- Go to we local Barangay Hall.
- Request and fill out the barangay clearance form.
- Present valid ID and supporting documents (e.g., proof of residence, business documents).
- Pay the clearance fee.
- Barangay staff will verify residency and records.
- Receive the signed barangay clearance immediately or within the day.
Fishery Registration
Fishing Vessels, Licenses
- Visit the Municipal Agriculture Office.
- Fill out the registration form for fishing vessel or license.
- Submit requirements (proof of ownership, boat specifications, fisherfolk ID).
- Pay the registration/license fee.
- Undergo inspection if necessary.
- Receive the fishing license or registration certificate.
Health Services
Consultations, Vaccinations, Maternal Care
- Go to the Municipal Health Office or designated health center.
- Register at the reception desk.
- Present valid ID or barangay certificate if required.
- State the service needed (consultation, vaccination, maternal care, medical assistance).
- Wait for we turn with the attending physician/nurse.
- Receive the service or prescription.
- For medicines or assistance, proceed to the pharmacy/assistance desk.
